Vendor Hazardous Goods Declaration Form Please refer to the following form related to hazardous products that you may manufacture or sell. This information is vital to our Retail Support Centers as it allows them to establish appropriate bin locations and appropriate handling of any hazardous prod Also included is a letter outlining the requirements of California Prop 65. Please review the information, sign and return the statement to your Ace buyer. Dear Manufacturer: Ace Hardware Corporation manages the way in which we transport our hazardous materials to ensure compliance with all applicable hazardous materials regulations, especially those that relate to international shipments. Enclosed is a Hazardous Goods Product Information Request Form. This form requests specific shipping information about the products that Ace Hardware Corporation purchases from your company. This information is required for domestic ground, vessel and commercial air transportation. The requested information will be entered into a central database maintained by Professional Emergency Resource Services (PERS). Please complete a Hazardous Goods Product Information Request Form for each of your hazardous products. Be certain to complete a separate form for each UPC (package type, size or volume). In addition to the product information request form, the most recent version of MSDS for each product will be required. If your company offers access to your MSDS on the internet, please provide the URL. It is imperative that both your stock number and the associated Ace Hardware SKU are annotated on the MSDS and the request form is attached to the appropriate MSDS to ensure accurate classification for each product. The Hazardous Goods Product Information Request Form(s) and applicable MSDS' should be sent directly to PERS. In order to ensure a continued business relationship, we urge you to give this important matter your prompt attention.
